Release and Waiver
This Agreement is made between the parent or guardian and GRTKD II, LLC, hereinafter referred to as “GRTKD". GRTKD agrees to provide expert professional instruction in Taekwon-do, and to assist the student in attaining his/her maximum potential. In return, the student is expected to practice with diligence and sincerity.
The parent or guardian agrees on the student's behalf to have photographs and video taken, and the images become the property of GRTKD. The parent or guardian further gives permission for these images to be used in advertising, brochures or other forms of media without compensation.
The parent or guardian and student are also expected to follow the school rules and agree not to endanger or threaten the safety of others. This includes the use of protective gear required by GRTKD. GRTKD does not warrant any protective gear.
The parent or guardian also understands that GRTKD does not pay for the treatment of any injuries.
The parent or guardian and student, on behalf of ourselves, members of our family, our heirs, executors, administrators and assigns, hereby accepts full responsibility for the use of, or participation in, any and all classes, services, equipment, demonstrations or events, whatsoever owned, operated or sponsored by GRTKD, whether on-site or off-site and hereby forever release, discharge and hold harmless GRTKD, its owners, instructors, members, employees, representatives, agents, Taekwon-do International and affiliates from any and all loss, claim, injury, damage or liability sustained or incurred by the student how so ever caused, arising out of or in connection with my taking part in martial arts classes and activities and not withstanding that the same may have been contributed to or occasioned by the negligence of GRTKD representatives or agents.
By agreeing to this Agreement, the parent or guardian certifies that the student is not suffering from any illness, physical or mental, which could inhibit the student’s ability to safely practice Taekwon-do. The parent or guardian hereby certifies that the student is physically fit and capable of participating in the school’s program.
The parent or guardian recognizes that regular attendance is important to the student’s progress and understands that it is the student’s responsibility to attend class. If the student foresees missing more than two weeks, it is his/her responsibility to notify GRTKD.
The parent or guardian agrees that the cost of uniforms, test fees, tournament fees, protective gear, private classes, and other GRTKD related activities are not covered by this document and remain the responsibility of the parent or guardian.
The parent or guardian acknowledges that they have read each and every paragraph of this Agreement and fully understands the same,and that the parent or guardian is not suffering from any mental or physical disability that will prevent them from understanding the same.
